Pre-Participation Forms
Bishop McHugh students wishing to participate in sports must be given a pre-participation physical examination pursuant to N.J.A.C. 6A:16. Physical exams are good for one calendar year. The Athletic Health Form Packet is required for grades 6-8 and requested for grades 5 and below. The History form (first page in the Athletic Form Packet) is to be filled out by the parent/ guardian prior to the child seeing the physician. The History form must be updated every 90 days via the parent/ guardian. In other words, even if your child has had a physical within the past year, please complete and provide the school with an updated History form prior to the upcoming sport season. This alerts us to any medical updates since the last physical.
Only the History form, the Physical exam forms and signature forms (e.g., Concussion Fact Sheet, etc.) are to be turned in. Informational pamphlets, such as Sports Related Eye Injuries, Opioid Use and Misuse Educational Fact Sheet and Cardiac Information are yours to keep.
Please fill out ALL paperwork completely and legibly so that it can be processed in an organized fashion. Incomplete forms cannot be accepted and may result in your child being ineligible.
Thank you for your understanding!
