Bishop McHugh students wishing to participate in sports must be given a pre-participation physical examination pursuant to N.J.A.C. 6A:16. Physical exams are good for one calendar year. The Athletic Health Form Packet is required for grades 6-8 and requested for grades 5 and below. The NJ Health History Form is to be filled out by the parent/ guardian prior to the child seeing the physician. The Health History Form must be updated every 90 days via the parent/ guardian.
